Esse variano ampiamente in termini di eziologia, forma e dimensioni e possono essere riscontrate in tutto il percorso endodontico.Il trattamento delle perforazioni iatrogene prevede una serie di alternative terapeutiche, le cui indicazioni dipendono da determinati criteri. Va notato che il trattamento endodontico ortogrado dovrebbe essere preferito.Lo scopo di questo articolo è quello di guidare il medico verso il necessario approccio terapeutico alla perforazione dentale iatrogena, sottolineando l'importanza della prevenzione. Inoltre, fa luce sui biomateriali di riparazione appropriati che possono migliorare la prognosi di questo tipo di incidente.

Iatrogene Zahnperforationen sind die zweithäufigste Ursache für endodontisches Versagen, da sie eine Verbindung zwischen dem endodontischen Raum und dem Parodont herstellen. Sie sind in Bezug auf ihre Ätiologie, Form und Größe sehr vielfältig und können im gesamten endodontischen Pfad auftreten.Bei der Behandlung von iatrogenen Perforationen kommen zahlreiche Behandlungsalternativen zum Einsatz, die ihre Indikationen nach bestimmten Kriterien finden. Es ist zu beachten, dass die orthograde Behandlung auf endodontischem Weg zu bevorzugen ist.Diese Arbeit soll den Zahnarzt auf den notwendigen therapeutischen Ansatz bei einer iatrogenen Zahnperforation hinweisen und gleichzeitig die Bedeutung der Prävention hervorheben. Sie beleuchtet auch die geeigneten Reparatur-Biomaterialien, die die Prognose eines solchen Unfalls verbessern können.

As perfurações dentárias iatrogénicas são a segunda causa mais comum de insucesso endodôntico, criando comunicação entre o espaço endodôntico e o periodonto. Elas variam muito em termos de etiologia, forma e tamanho, e podem ser encontradas em todo o percurso endodôntico.O tratamento das perfurações iatrogénicas envolve várias alternativas terapêuticas, cujas indicações dependem de determinados critérios. É importante ressaltar que o tratamento endodôntico ortógrado deve ser privilegiado.O objetivo deste artigo é orientar o profissional para a abordagem terapêutica necessária à perfuração dentária iatrogénica, salientando a importância da prevenção. Também esclarece sobre os biomateriais de reparação adequados que podem melhorar o prognóstico deste tipo de acidente.

Iatrogenic dental perforations are the second most common cause of endodontic failure, creating communication between the endodontic space and the periodontium. They are highly diverse in terms of etiology, shape and size, and can occur anywhere along the endodontic path.The management of iatrogenic perforations calls for a number of therapeutic alternatives, the indications for which depend on certain criteria. Orthograde endodontic treatment is the preferred option.The aim of this paper is to guide the practitioner towards the necessary therapeutic approach to iatrogenic dental perforation, while emphasizing the importance of prevention. It also sheds light on the appropriate repair biomaterials that can improve the prognosis of such an accident.

La frattura strumentale in endodonzia è una complicazione che impedisce il completamento del trattamento endodontico. Il medico può avere a disposizione diverse opzioni di trattamento. La gestione della frattura strumentale deve basarsi su una conoscenza approfondita delle percentuali di successo di ciascuna opzione terapeutica.La gestione delle fratture strumentali va da un approccio conservativo ortogrado, rappresentato essenzialmente dalla rimozione o dal bypass, a un approccio chirurgico più radicale e invasivo, come la resezione apicale o l'amputazione della radice, che dovrebbe essere indicato solo come ultima risorsa.Esiste un'ampia gamma di kit per la rimozione, compresi gli estrattori tubolari, e alcuni professionisti utilizzano persino tecniche fatte in casa con file H o aghi da siringa. Management of instrumental fracture should be based on a thorough understanding of the success rates of each treatment option.The management of instrumental fractures ranges from a conservative orthograde approach represented essentially by removal or bypass, to a more radical and invasive surgical approach such as apical resection or root amputation, which should be indicated only as a last resort.A wide range of removal kits are available, including tube extractors, and some practitioners even use homemade techniques using H-files or syringe needles. However, when properly mastered, the use of ultrasound has proved to be the safest and most effective method for instrument removal. The aim of this paper is to detail the management of instrument fractures in endodontics.
