The management of distal tibial fractures is a challenge for surgeons. We report the results of our experience with minimally invasive osteosynthesis of these fractures using the latest generation of Synthes® locking screw plates. This was a prospective study of 13 patients. All were closed fractures associated with a distal fibular fracture. Osteosynthesis was performed on a reduced fracture using traction on an orthopaedic table or an external fixator. All fibular fractures were osteosynthesised. Primary osteosynthesis of these fibular fractures was performed in 61.53% of cases. Osteosynthesis is feasible if reduction criteria are met before synthesis. No complications were observed apart from 2 malunion fractures, with no functional consequences, which occurred at the beginning of our experience. Patients were satisfied in 84.61% of cases and returned to work. Minimally invasive osteosynthesis using a plate with locked screws ensures sufficient and rapid autonomy with fewer complications.

A gestão das fracturas da tíbia distal é um desafio para os cirurgiões. Relatamos os resultados da nossa experiência com a osteossíntese minimamente invasiva destas fracturas, utilizando a última geração de placas de parafusos de bloqueio Synthes®. Este foi um estudo prospetivo de 13 pacientes. Todos eram fracturas fechadas associadas a uma fratura distal da fíbula. A osteossíntese foi realizada numa fratura reduzida com tração numa mesa ortopédica ou num fixador externo. Todas as fracturas da fíbula foram osteossintetizadas. A osteossíntese primária destas fracturas fibulares foi realizada em 61,53% dos casos. A osteossíntese é viável se os critérios de redução forem cumpridos antes da síntese. Não foram observadas complicações, à exceção de 2 fracturas malunionadas, sem consequências funcionais, que ocorreram no início da nossa experiência. Os pacientes ficaram satisfeitos em 84,61% dos casos e regressaram ao trabalho. A osteossíntese minimamente invasiva com uma placa com parafusos bloqueados garante uma autonomia suficiente e rápida com menos complicações.

La gestione delle fratture tibiali distali è una sfida per i chirurghi. Riportiamo i risultati della nostra esperienza con l'osteosintesi mini-invasiva di queste fratture utilizzando l'ultima generazione di placche a vite di bloccaggio Synthes®. Si tratta di uno studio prospettico su 13 pazienti. Tutte le fratture erano chiuse e associate a una frattura del perone distale. L'osteosintesi è stata eseguita su una frattura ridotta utilizzando una trazione su un tavolo ortopedico o un fissatore esterno. Tutte le fratture del perone sono state osteosintetizzate. L'osteosintesi primaria di queste fratture fibulari è stata eseguita nel 61,53% dei casi. L'osteosintesi è fattibile se i criteri di riduzione sono soddisfatti prima della sintesi. Non sono state osservate complicazioni, a parte 2 fratture malunite, senza conseguenze funzionali, che si sono verificate all'inizio della nostra esperienza. I pazienti sono stati soddisfatti nell'84,61% dei casi e sono tornati al lavoro. L'osteosintesi mini-invasiva con placca e viti bloccate garantisce un'autonomia sufficiente e rapida con minori complicanze.

Die Behandlung von distalen Tibiafrakturen stellt eine Herausforderung für Chirurgen dar. Wir berichten über die Ergebnisse unserer Erfahrung mit der minimalinvasiven Osteosynthese dieser Frakturen mit der neuesten Generation von Synthes® Locked Screw Plate. Es handelte sich um eine prospektive Studie mit 13 Patienten. Es handelte sich ausnahmslos um geschlossene Frakturen in Verbindung mit einer distalen Fibulafraktur. Die Osteosynthese wurde an einer reduzierten Fraktur unter Zug auf einem orthopädischen Tisch oder mit einem Fixateur externe durchgeführt. Die Fibulafrakturen wurden alle osteosynthetisiert. Es traten keine Komplikationen auf, mit Ausnahme von zwei bösartigen Kallusbildungen ohne funktionelle Folgen, die zu Beginn unserer Erfahrung auftraten. Die Patienten waren in 84,61% der Fälle zufrieden und hatten ihre berufliche Tätigkeit wieder aufgenommen. Die minimalinvasive Osteosynthese mit einer Platte mit verriegelten Schrauben gewährleistet eine ausreichende, schnelle Unabhängigkeit mit weniger Komplikationen.
